Application for Funds Support Services for Deaf and Hard of Hearing Adults
Use this application to request funds authorized under Minnesota Statutes, section 124D.57 for interpreter and note-taker services for deaf, deafblind and hard-of-hearing adults. Only the authorized fiscal agent indicated in the “Applicant Agency and Program Information” section should submit this application. Please print or type entries and complete all information requested. Please read the instructions when entering data in the areas below. Submit form in duplicate for approval to the above address. NOTE: Final claim must be submitted by July 15 for the prior fiscal year.

HEAR NOW Foundation - Starkey Hearing Foundation
Hear Now is an application based program that provides hearing help to low-income Americans.  Each person is fit with new, top-of-the-line digital hearing aids that are customized to their hearing loss.  Call or email if you know someone is struggling with hearing loss and unable to afford hearing aids.

Minnesota Department of Human Services
The DHS provides mental health services and information to help Minnesotans who are deaf, deafblind and hard of hearing and their families.  Six locations throughout Minnesota.

Minnesota Department of Human Services - Telephone Equipment Distribution Program (TED)
The TED Program provides telephone equipment to people who are deaf, hard of hearing, deaf blind, speech impaired or have a physical disability and need adaptive equipment in order to use the phone. The equipment is loaned out at no cost as a long-term loan. Available equipment include Captel phones, light flashing ring signalers, TTYs, amplified telephones, loud ringers, hands free speakerphones and more.
